KYU GRADING EXAMINATION FORM

This form can keep a record of up to 9 Kyu Gradings.
It is formatted as a double sided form, for duplex printing.

KYU GRADING REPORT FORM

This form must be used for sending Kyu Grading Reports to the S.K.I. Australia Memberships Office.

APPLICATION TO DO DAN TEST FORM

This form must be completed by the Dojo Resident Instructor.

This is both an Examination Form and a Registration Form. It must be completed if you wish to register a Dan Grade from an organization other than the Shotokan Karate-do International Federation. Photographs are not required and a photocopy of your current Dan Certificate must be forwarded with the completed form. A registration fee will apply.
